File a Complaint

We strongly encourage investors to file their complaint using NFA's online File A Complaint Form below. If you prefer to file your complaint offline, you may download the printable complaint form and fax or mail it to NFA at the address noted at the end of the form. The printable complaint form is a Portable Document Format (PDF) file. If you do not have the Adobe® Acrobat Reader to view PDF file, please download the free plug-in. Please note that the online File A Complaint system is not available Monday through Friday from 8:00 PM to 9:00 PM CT. The system may also occasionally be unavailable at other times due to system maintenance.
