Must NFA FCM/FDM Members submit the assessment fee worksheet and Assessment Fee Summary Report with their payment?

NFA will accept assessment fee formats which vary from the worksheet submitted to Members for their convenience. Members should be able to adequately justify and document reasons for variation from the worksheet. All FCMs/FDMs are required, however, to submit the Assessment Fee Summary Report without modification.
